To obtain an IDEA Replacement card, please fill out this form as completely as possible, send in a 2 x 2 passport color photograph, and the appropriate processing fees. If you are not sure that you were certified through IDEA, please contact Headquarters at info@ideascuba.com before sending in this form.
After any period of inactivity, you may lose proficiency in your skills and knowledge and thus create an unsafe condition when you resume diving. For this reason, IDEA always recommends a refresher course.
